Download Enrollment Forms: Take a few minutes to download the necessary enrollment forms below.
Complete Forms: Once downloaded, please print out the forms and fill them out completely.
Submit Forms: Deliver the completed forms to the school office your child will attend. If you have any questions while filling out the forms, please don't hesitate to contact the school office at the appropriate building.

  - Please hand deliver (not mail) these forms to the school, along with:

Birth certificate

Proof of residency

Immunization records

Any other documentation you feel is helpful (i.e. IEP for Special Education students)

- - Additional paperwork is required for Special Education student enrollment
